One Price Prescription Drug Plan

The Lakeview Pharmacy One Price Prescription Drug Plan includes over 450 drugs, which allows you to purchase many of the top generic prescriptions in quantities of 90, or 100 tablets at a time. There is also a discount available for up to 8% off on all brand name drugs and 30% off on generics not included in the One Price Program brochure. The One Price Generic Drug Program is designed to combat the rising cost of medications and for Part D use while in the Donut Hole or to avoid reaching the Donut Hole. Many popular medications are included and our goal is to continually review the list to determine whether additional medications can be added.

You can benefit from this plan whether you have insurance or not, if you are on Medicare Part D or if your insurance demands you use mail order for your prescriptions.

Unfortunately, from time to time, we have to remove a drug from our One Price Program due to pricing.  Click here to view the list of drugs that have been removed since the last printing of the brochure.

To take advantage of the One Price Prescription Drug Plan, click on the button to the right to get the enrollment form. Print out the form and send it or take it to: Lakeview Pharmacy, 516 Monument Square, Racine, WI 53403. You will receive a One Price card in the mail. Use it every time you fill a prescription at Lakeview Pharmacy to automatically receive the benefit of lower drug costs. If you refill your prescriptions online or by phone, your card will be stored in our electronic files and you will still receive this benefit.
